When the decision is made to refer the application to the full IRB, the Chair or designee gives the reason(s) on the review form.<\/p><h3 class=\"wp-block-heading\" id=\"h-a-exempted-from-review\"><a id=\"a\" name=\"a\"><\/a>A. Exempted from Review<\/h3><p>Determinations regarding exemption from IRB review are made by the Chair or designee and not by investigators. Investigators can apply for an exemption by completing an exemption form.&nbsp;In making determinations on exemptions, the Chair or designee evaluates whether the narrative description of the research is consistent with the categories for exemption contained in federal and state regulations.<\/p><p>Exemptions are in force for a maximum of five years after approval.&nbsp;To continue research after that time period, investigators must apply for another exemption or complete a regular IRB application.<\/p><h5 class=\"wp-block-heading\" id=\"h-1-exempt-categories\"><a id=\"a1\" name=\"a1\"><\/a>1. Exempt Categories<\/h5><p><span style=\"font-weight: bold;\">Category a.<\/span> The research must be conducted in a commonly accepted educational setting (e.g., classroom, conference facility) as opposed to a laboratory or similar setting <u>and<\/u> involve commonly accepted educational practices (e.g., forms of instructional methods) as opposed to psychological manipulations or non-instructional interventions.<\/p><p><span style=\"font-weight: bold;\">Category b<\/span>. The research (educational tests, surveys, interviews, observations of public behavior) is conducted anonymously (names and identifying information are not recorded) <u>and<\/u> involves the collection of benign information that could not be damaging to a person. &ldquo;Public behavior&rdquo; is defined as behavior in places that any person can visit without special permission (e.g., street corners, parks, restaurants, theaters) or Internet communications in public forums. Observations of the public behavior of children do not qualify for exemption if the investigator interacts with them.<\/p><p><span style=\"font-weight: bold;\">Category c. <\/span>The research (educational tests, surveys, interviews, or observations of public behavior) is conducted among elected or appointed public officials or candidates for office.<\/p><p><span style=\"font-weight: bold;\">Category d.&nbsp;<\/span>The research involves the study of data (in existence prior to the research) that are publicly available <u>or<\/u> that are recorded in a manner in which they cannot be linked with individuals (e.g., there is not a master list of subjects that links participants to identification numbers).<\/p><p><span style=\"font-weight: bold;\">Category e. <\/span>The research to be conducted has been approved by federal departments or agency heads and determined to be exempt by those departments and agencies. To qualify for exemption under this category, investigators would be required to provide the IRB with appropriate documentation from the relevant federal departments or agencies.<\/p><p><span style=\"font-weight: bold;\">Category f.<\/span>&nbsp;Research involving food (taste, quality, or consumer acceptance) which is wholesome and contains no additives <u>or<\/u> foods with ingredients (type and amount) that meet federal safety standards.<\/p><h5 class=\"wp-block-heading\" id=\"h-2-responsibilities-of-investigators-conducting-exempt-research\"><a id=\"a2\" name=\"a2\"><\/a>2.&nbsp;Responsibilities of Investigators Conducting Exempt Research<\/h5><p>Since exempted studies are still considered human subjects research, investigators are expected to adhere to ethical standards for the conduct of research.&nbsp;Specifically, investigators are expected to obtain Informed Consent Releases (when appropriate, given the nature of the research), take all necessary steps to minimize risks and maximize research benefits, and ensure the equitable selection of participants (when warranted). Investigators of exempt studies may not make substantive changes in the design of their studies, including changes involving investigators, research instruments and procedures, the location of their research, and sources of participants or data, without written approval by the IRB before any such changes are made. Changes must be requested in writing.<\/p><h3 class=\"wp-block-heading\" id=\"h-b-expedited-review\"><a id=\"b\" name=\"b\"><\/a>B. Expedited Review<\/h3><p>Research activities that present no more than minimal risk <u>may<\/u> be eligible for expedited IRB approval. &nbsp;Determinations regarding expedited IRB approval are made by the Chair or his\/her designee. The Chair or designee may seek the advice of another IRB member or consultant who has specific expertise in the area of research described in the application.<\/p><p>For purposes of Expedited Review, minimal risk refers to the risks of everyday life (e.g., providing information to others about one&rsquo;s attitudes and other non-confidential issues) or those associated with routine medical and psychological tests.&nbsp;The expedited review procedure may not be used where identification of participants or their responses would reasonably place them at risk of criminal or civil liability or be damaging to their financial standing, employability, insurability, reputation, or be stigmatizing unless reasonable protections will be implemented to minimize these risks.&nbsp;Expedited approval may not be given for classified research.<\/p><h5 class=\"wp-block-heading\" id=\"h-1-overview-of-expedited-research\"><a id=\"b1\" name=\"b1\"><\/a>1.&nbsp;Overview of Expedited Research<\/h5><p>Research activities that (1) present no more than minimal risk to human participants and, (2) involve only procedures listed in one or more of the following seven categories may be reviewed by the IRB through the expedited review procedure.&nbsp;The activities listed are not deemed to be of minimal risk simply because they are included on this list; rather, inclusion on this list means that the activity is eligible for expedited review because the specific circumstances of the proposed research are seen to involve no more than minimal risk to human participants.<\/p><p>Categories 1 &ndash; 7 below apply regardless of the age of participants, except as noted.&nbsp;These seven categories pertain to both initial and continuing IRB review.<\/p><p>Standard requirements for informed consent (or its waiver, alteration, or exception) apply regardless of the type of review&ndash;Expedited or Full Board&ndash;utilized by the IRB.<\/p><h5 class=\"wp-block-heading\" id=\"h-2-expedited-review-categories\"><a id=\"b2\" name=\"b2\"><\/a>2. Expedited Review Categories<\/h5><p><span style=\"font-weight: bold;\">Category 1.<\/span> Clinical studies of drugs and medical devices only when condition (a) or (b) is met.&nbsp; (a) &nbsp;Research on drugs for which an investigational new drug application (21 CFR Part 312) is not required. &nbsp;(<u>Note<\/u>: Research on marketed drugs that significantly increases the risks or decreases the acceptability of the risks associated with the use of the product is not eligible for Expedited Review.)&nbsp; (b) &nbsp;Research on medical devices for which (i) an investigational device exemption application (21 CFR Part 812) is not required; or (ii) the medical device is cleared\/approved for marketing and the medical device is being used in accordance with its cleared\/approved labeling.<\/p><p><span style=\"font-weight: bold;\">Category 2.<\/span> Collection of blood samples by finger stick, heel stick, ear stick, or venipuncture as follows:&nbsp; (a) &nbsp;from healthy, nonpregnant adults who weigh at least 110 pounds.&nbsp;For these participants, the amounts drawn may not exceed 550 ml in an eight (8) week period and collection may not occur more frequently than two (2) times per week; <u>or<\/u> (b) &nbsp;from other adults and children, considering the age, weight, and health of the participants, the collection procedure, the amount of blood to be collected, and the frequency with which it will be collected. &nbsp;For these participants, the amount drawn may not exceed the lesser of 50 ml or three (3) ml per kg in an eight (8) week period and collection may not occur more frequently than two (2) times per week.<\/p><p><span style=\"font-weight: bold;\">Category 3.<\/span>&nbsp;Prospective collection of biological specimens for research purposes by noninvasive means.&nbsp; Examples:&nbsp; (a) hair and nail clippings in a nondisfiguring manner; (b) deciduous teeth at time of exfoliation or if routine patient care indicates a need for extraction; (c) permanent teeth if routine patient care indicates a need for extraction;&nbsp;(d) excreta and external secretions (including sweat); (e) uncannulated saliva collected either in an unstimulated fashion or stimulated by chewing gumbase or wax or by applying a dilute citric solution to the tongue; (f) placenta removed at delivery; (g) amniotic fluid obtained at the time of rupture of the membrane prior to or during labor; (h) supra- and subgingival dental plaque and calculus, provided the collection procedure is not more invasive than routine prophylactic scaling of the teeth and the process is accomplished in accordance with accepted prophylactic techniques; (i) mucosal and skin cells collected by buccal scraping or swab, skin swab, or mouth washings; (j) sputum collected after saline mist nebulization.<\/p><p><span style=\"font-weight: bold;\">Category 4. <\/span>Collection of data through noninvasive procedures (not involving general anesthesia or sedation) routinely employed in clinical practice, excluding procedures involving x-rays or microwaves. &nbsp;Where medical devices are employed, they must be cleared\/approved for marketing. &nbsp;(Studies intended to evaluate the safety and effectiveness of the medical device are not generally eligible for expedited review, including studies of cleared medical devices for new indications.) <u>Examples<\/u>: (a) physical sensors that are applied either to the surface of the body or at a distance and do not involve input of significant amounts of energy into the particpant or an invasion of the participant&rsquo;s privacy; (b) determining body weight or testing sensory acuity;&nbsp;(c) magnetic resonance imaging; (d) electrocardiography, electroencephalography, thermography, detection of naturally occurring radioactivity, electroretinography, ultrasound, diagnostic infrared imaging, doppler blood flow, and echocardiography;&nbsp;(e) moderate exercise, muscular strength testing, body composition assessment, and flexibility testing where appropriate given the age, weight, and health of the individual.<\/p><p><span style=\"font-weight: bold;\">Category 5.<\/span>&nbsp;Research involving materials (data, documents, records, or specimens) that have been collected, or will be collected solely for nonresearch purposes (such as medical treatment or diagnosis).<\/p><p><span style=\"font-weight: bold;\">Category 6.<\/span>&nbsp;Collection of data from voice, video, digital, or image recordings made for research purposes.<\/p><p><span style=\"font-weight: bold;\">Category 7. <\/span>Research on individual or group characteristics or behavior (including, but not limited to, research on perception, cognition, motivation, identity, language, communication, cultural beliefs or practices, and social behavior) or research employing survey, interview, oral history, focus group, program evaluation, human factors evaluation, or quality assurance methodologies.<\/p><h3 class=\"wp-block-heading\" id=\"h-c-nbsp-full-board-review\"><a id=\"c\" name=\"c\"><\/a>C. &nbsp;Full Board Review<\/h3><p>Human research not eligible for Exemption or Expedited Review is evaluated by the IRB at a convened meeting. A majority of the IRB members, including at least one member whose primary concerns are not scientific, must be present to conduct a convened meeting.&nbsp;For research to be approved, it must receive the approval of a majority of those members present at the meeting where a quorum is in attendance.<\/p><p>Convened meetings are scheduled at least once a semester.&nbsp;The deadline for submission of materials for full IRB review is 10 workdays prior to the scheduled meeting. IRB members are sent applications and other materials for review immediately after the deadline for submission.&nbsp;Members are expected to review all materials prior to the meeting.<\/p><p>IRB meetings are conducted by the Chair or designee. The Chair presents an initial review of the research proposal, indicating preliminary questions or concerns, if any, about the proposal followed by a discussion among all IRB members present. &nbsp;In its review, the IRB considers the following review criteria: (1) adequacy and completeness of the application, (2) research design, (3) participant selection, (4) risks and benefits, (5) minimization of risks, (6) privacy and confidentiality, (7) additional safeguards for vulnerable participants, (8) consent, and (9) additional considerations.&nbsp;Whenever possible, the IRB attempts to reach consensus on reviewed research. IRB decisions are recorded as unanimous when consensus has been achieved. When members are unable to reach consensus on a decision, the decision is made by majority vote, with members voting for approval or disapproval (or abstaining), or required changes, as identified in the minutes.<\/p><p>The IRB makes one of four decisions on reviewed research:&nbsp;(1) Approval, (2) Provisional Approval (the IRB authorizes the Chair or another member to review and approve required revisions), (3) Postponed (tabled, pending further information), or (4) Withheld (disapproved). In a case where the research proposal is not fatally flawed or devoid of merit, but might be able to be strengthened, the IRB may provide recommendations or suggestions to investigators to improve their research design or methodology.<\/p><p>The Chair may permit persons not affiliated with the IRB (e.g., consultants) to attend meetings, upon request. &nbsp;Investigators or their collaborators are not permitted to be present at IRB meetings during actual deliberations on their research proposals. Its primary responsibility is to protect the welfare of human participants in research and, to this end, the IRB reviews and monitors all research involving humans conducted at the University.&nbsp;The role of the IRB is to ensure that researchers meets current standards and mandates regarding ethical treatment of research participants and to report any problems to the Office of the Associate Provost.<\/p><h3 class=\"wp-block-heading\" id=\"h-b-scope-of-the-irb\"><a id=\"b\" name=\"b\"><\/a>B. Scope of the IRB<\/h3><p>The IRB is responsible for all research conducted at UMW by any University employee, student, or agent either in the course of his or her University responsibilities or when using the University&rsquo;s name, symbols, property, or services in connection with the research. No research using human participants may be initiated without prospective approval by the IRB.&nbsp;The IRB has the authority to approve, require modifications to, or disapprove of, proposed human subjects research.&nbsp;The IRB also has the authority to halt or suspend research when necessary to protect the welfare of research participants. The University&rsquo;s IRB has no responsibility for, or authority over, research conducted outside the University by individuals not affiliated with the University of Mary Washington.<\/p><h5 class=\"wp-block-heading\" id=\"h-1-covered-research\"><a id=\"b1\" name=\"b1\"><\/a>1.&nbsp;Covered Research<\/h5><p>All research conducted by faculty, staff, and students in conjunction with their official duties at UMW is subject to IRB review.&nbsp;Some research conducted by students that is used to fulfill requirements for their degree programs must be approved by the IRB, with exceptions as noted below.&nbsp;Faculty and staff research must be approved by the IRB, <span style=\"text-decoration: underline;\">whether or not the research receives funding from external sources.<\/span><\/p><p>The IRB has no jurisdiction over research conducted by members of the university community which is not related to their official roles and responsibilities.&nbsp;Faculty, in particular, may undertake research projects on their own as private contractors. &nbsp;Such research will be deemed independent of their official roles and responsibilities and not subject to IRB review if:&nbsp;(1) the research is conducted on their own time and not reimbursed by the university or through university accounts, (2) the research is conducted without the use of university space, materials, and supplies, and secretarial or staff support, and (3) any research publications, reports, or presentations will not list the faculty member&rsquo;s position and affiliation with the University of Mary Washington.<\/p><p>IRB approval is not required for members of the University who act as consultants on human subjects research that involves participants not affiliated with UMW.<\/p><h5 class=\"wp-block-heading\" id=\"h-2-research-conducted-at-umw-by-investigators-from-other-institutions\"><a id=\"b2\" name=\"b2\"><\/a>2.&nbsp;Research Conducted at UMW by Investigators from Other Institutions<\/h5><p>UMW faculty and staff are sometimes approached by investigators at other institutions for cooperation in the latter&rsquo;s research. For example, department chairs or deans might be asked to assist in the distribution of surveys to faculty or students. This research does not fall under the jurisdiction of the IRB unless:&nbsp;(1) University facilities, including physical space and resources will be used, (2) University officials are actively engaged in or actively cooperate with or encourage participation in the research, or (3) University faculty, staff, or students intend to use the findings or results of these studies for their own purposes.<\/p><p>Approval by the IRB is not required by University officials who wish to inform members of the University community about research or provide them with information about contacting investigators if the latter wish to participate in such research.<\/p><h5 class=\"wp-block-heading\" id=\"h-3-student-projects-research-vs-pedagogy\"><a id=\"b3\" name=\"b3\"><\/a>3.&nbsp;Student Projects: Research vs. Pedagogy<\/h5><p>Some courses at UMW require students to complete projects as a way of teaching them research methods and skills. Institutional Review Boards at institutions of higher education vary according to whether they require student projects to be reviewed and approved. The University of Mary Washington&rsquo;s IRB does not require student projects conducted in research methods courses to be reviewed if the purpose of these projects is pedagogical in nature. Activities not intended to provide generalizable knowledge are not subject to IRB review. Although the IRB does not require review of such projects, it assumes that instructors in these courses provide close supervision of students, teach students the ethics of human research, and ensure that students conduct these projects in a manner consistent with the precepts of the Nuremberg Code and the Belmont Report (cf. pp. 1-2 and the Appendices section of this Manual).<\/p><p>Note. &ndash; <u>Without<\/u> IRB approval, students are <u>not<\/u> permitted to continue projects conducted for a research methods course after the semester has ended.<\/p><p>Students in graduate courses, in particular, sometimes use projects to refine their research interests and provide a foundation for a thesis. In order to use these data for theses, or other research purposes, students must either: (1) demonstrate that participants provided informed consent for the project at the time, through procedures approved by the instructor, or (2) obtain consent from people to use previously collected information according to procedures approved by the IRB (i.e., an application for Exemption, Expedited Review, or Full Board Review).<\/p><p>Student projects in courses are subject to IRB scrutiny if they are designed at least partially to provide data for research purposes. For example, instructors may enlist students to assist in data collection or analysis for their own research or may design seminars in which a goal is for students to collaborate on research that will be submitted for publication. These projects constitute research and must be submitted to the IRB for approval before any actual research activity commences.<\/p><h5 class=\"wp-block-heading\" id=\"h-4-grant-applications\"><a id=\"b4\" name=\"b4\"><\/a>4. Grant Applications<\/h5><p>The IRB requires an investigator submitting a proposal for externally funded human subjects research to have the proposal evaluated and approved by the IRB <u>prior<\/u> to such submission. This approval is necessary because external funding agencies often require human subjects research to be approved by the IRB prior to the awarding of a grant.<\/p><h5 class=\"wp-block-heading\" id=\"h-5-pilot-studies\"><a id=\"b5\" name=\"b5\"><\/a>5. Pilot Studies<\/h5><p>Investigators sometimes conduct pilot studies designed to develop or refine research procedures and instruments.&nbsp;Although data collected through pilot studies may not ultimately be used in research reports and publications, pilot studies represent part of the research process that leads to the development of, or contribution to, generalizable knowledge. As such, pilot studies require IRB review and approval before they are conducted.<\/p><h5 class=\"wp-block-heading\" id=\"h-6-evaluations\"><a id=\"b6\" name=\"b6\"><\/a>6. Evaluations<\/h5><p>Evaluations are subject to IRB review if they fit the definition of research provided above:&nbsp;i.e., a systematic investigation designed to contribute to generalizable knowledge. Not all evaluations fit this definition. Evaluations conducted <u>exclusively<\/u> for quality assurance, quality improvement, or accountability purposes are not considered research in the current context and do not require IRB review. In these evaluations, there is no intention to share knowledge and information with external audiences.&nbsp;By way of example, a faculty member conducting a routine course evaluation or a department surveying the satisfaction of graduates to improve program quality would not be required to go through the IRB; however, a faculty member or administrator evaluating a program or teaching strategy with a view towards reporting the results professionally would be required to have the evaluation process approved by the IRB.<\/p><h3 class=\"wp-block-heading\" id=\"h-c-i-nstitutional-oversight-of-the-irb\"><strong><a id=\"c\" name=\"c\"><\/a>C. I<\/strong>nstitutional Oversight of the IRB<\/h3><p>The Associate Provost for Institutional Analysis and Effectiveness, who reports to the Provost, is responsible for the oversight of all human subjects research conducted at the University.&nbsp;The UMW IRB reports directly to the Associate Provost. The Associate Provost is the institutional official listed on UMW&rsquo;s Federal Wide Assurance and is ultimately responsible for overseeing the protection of human subjects in research conducted at UMW.&nbsp;Any serious risks or unanticipated problems encountered by the IRB must be reported to the Associate Provost. The Associate Provost is also responsible for developing and administering necessary educational programs to meet the requirements set forth in this Manual.<\/p><h3 class=\"wp-block-heading\" id=\"h-d-irb-membership\"><a id=\"d\" name=\"d\"><\/a>D. IRB Membership<\/h3><h5 class=\"wp-block-heading\" id=\"h-1-requirements\"><a id=\"d1\" name=\"d1\"><\/a>1. Requirements<\/h5><p>To be in compliance with Federal Regulations (45 CFR 46.107), the following requirements and conditions (D.1 a-h) must be met:<\/p><p>a) The IRB will have at least five members.<\/p><p>b)&nbsp;IRB members will represent varying backgrounds and disciplines.<\/p><p>c)&nbsp;IRB members will be diverse relative to race, gender, and cultural background.<\/p><p>d)&nbsp;IRB members will include persons able to ascertain the acceptability of proposed research in terms of institutional commitments, regulations, applicable law, and standards of professional conduct and practice.<\/p><p>e)&nbsp;The IRB will not consist entirely of members of one profession.<\/p><p>f)&nbsp;The IRB will contain at least one member who possesses expertise in a <u>scientific<\/u> area.<\/p><p>g)&nbsp;The IRB will have at least one member who possesses expertise in a <u>non-scientific<\/u> area.<\/p><p>h)&nbsp;The IRB will have at least one member who is not formally affiliated with UMW and is not an immediate family member of someone affiliated with the University of Mary Washington.<\/p><h5 class=\"wp-block-heading\" id=\"h-2-appointment-of-irb-members-length-of-service-and-duties\"><a id=\"d2\" name=\"d2\"><\/a>2.&nbsp;Appointment of IRB Members, Length of Service, and Duties<\/h5><p>IRB members at UMW are appointed by the Provost, serve staggered three year terms, and are eligible for reappointment. Members of the IRB are required to review research proposals and then vote to approve, modify, or not approve such proposals. Members are expected to attend IRB meetings, serve as primary reviewers in their respective areas of expertise, and serve as general reviewers for all research discussed at IRB meetings. Members may also conduct expedited reviews as appropriate when designated to do so by the IRB Chair.<\/p><h5 class=\"wp-block-heading\" id=\"h-3-appointment-of-chair-length-of-service-and-duties\"><a id=\"d3\" name=\"d3\"><\/a>3. Appointment of Chair, Length of Service, and Duties<\/h5><p>The Chair of the IRB is appointed by the Provost.&nbsp;Chairs serve 3-year, renewable terms.&nbsp; In addition to serving as a voting member of the IRB, the Chair is responsible for convening and conducting IRB meetings.&nbsp;The Chair works with IRB members, institutional officials, and investigators to make sure that applicable requirements are met.&nbsp;The Chair is responsible for signing off on all official IRB correspondence.<\/p><h5 class=\"wp-block-heading\" id=\"h-4-conflict-of-interest\"><a id=\"d4\" name=\"d4\"><\/a>4.&nbsp;Conflict of interest<\/h5><p>No IRB member may participate in the review of any proposed research project in which the member has a clear interest or involvement in said project.<\/p><h5 class=\"wp-block-heading\" id=\"h-5-use-of-consultants\"><a id=\"d5\" name=\"d5\"><\/a>5. Use of Consultants<\/h5><p>The IRB may, at its discretion, invite individuals with competence in special areas to assist in the review of issues that require expertise beyond, or in addition to, that possessed by members of the IRB. Consultants provide guidance and advice to the IRB but are not considered members of the Board.<\/p><h3 class=\"wp-block-heading\" id=\"h-e-training-of-irb-members\"><a id=\"e\" name=\"e\"><\/a>E.&nbsp;Training of IRB Members<\/h3><p>Appointed IRB members will receive comprehensive reference materials including educational information from the Office for Human Research Protections &nbsp;and this Manual.&nbsp;At a minimum, members will complete the tutorial available at the Office for Human Research Protections website or attend a workshop designed to train them to review research from an ethical and regulatory standpoint. Such a workshop would be given by a trained staff member from the Office of Provost.<\/p><h3 class=\"wp-block-heading\" id=\"h-f-administrative-support\"><a id=\"f\" name=\"f\"><\/a>F.&nbsp;Administrative Support<\/h3><h5 class=\"wp-block-heading\" id=\"h-1-budget\"><a id=\"f1\" name=\"f1\"><\/a>1. Budget<\/h5><p>Expenditures in support of IRB activities will be charged against the budget of the Office of the Provost<\/p><h5 class=\"wp-block-heading\" id=\"h-2-administrative-responsibilities-chair-of-the-irb\"><a id=\"f2\" name=\"f2\"><\/a>2. Administrative Responsibilities (Chair of the IRB)<\/h5><p>a) Scheduling and conducting IRB meetings.<\/p><p>b)&nbsp;Drafting correspondence to UMW investigators.<\/p><h5 class=\"wp-block-heading\" id=\"h-3-administrative-responsibilities-office-of-the-provost\"><a id=\"f3\" name=\"f3\"><\/a>3. Administrative Responsibilities (Office of the provost)<\/h5><p>a)&nbsp;Copying and distributing meeting materials (including minutes of IRB meetings).<\/p><p>b)&nbsp;Ensuring that new IRB members receive necessary training.<\/p><p>c)&nbsp;Serving as a resource for investigators.<\/p><p>d)&nbsp;Maintaining training and reference materials.<\/p><p>e)&nbsp;Maintaining and updating this Manual.<\/p><p>f)&nbsp;&nbsp;Drafting correspondence to government officials.<\/p><p>g) Filing Federal Wide Assurance documents.<\/p><h3 class=\"wp-block-heading\" id=\"h-g-record-keeping-and-documentation-office-of-the-provost\"><a id=\"g\" name=\"g\"><\/a>G.&nbsp;Record Keeping and Documentation (Office of the Provost)<\/h3><h5 class=\"wp-block-heading\" id=\"h-1-official-records-maintained-for-the-irb\"><a id=\"g1\" name=\"g1\"><\/a>1.&nbsp;Official Records Maintained for the IRB<\/h5><p>a)&nbsp;Written procedures and policies.<\/p><p>b)&nbsp;Maintaining official roster of IRB membership (including members&rsquo; names, earned degree(s) and specialty(-ies), and representative capacity).<\/p><p>c)&nbsp;Training records (of IRB members and researchers).<\/p><p>d) IRB correspondence.<\/p><p>e)&nbsp;Documentation of exemptions and exceptions.<\/p><p>f)&nbsp;Minutes of IRB meetings, which shall include:&nbsp; attendance (names of IRB members, researchers, consultants or guests present), actions taken by the IRB, the basis for requiring changes or disapproving research, a list of research projects approved since the last meeting that involved the Expedited Review option.<\/p><p>g)&nbsp;Reports of any adverse events.<\/p><p>h)&nbsp;IRB research proposal files.&nbsp;The IRB will maintain a file for each research application that it receives for review.&nbsp; Applications will be numbered sequentially by calendar year in the order in which they are received. &nbsp;(e.g., the first application in 2005 will be numbered 2005-001). Files will contain the application, documentation of type of IRB review, consent forms, advertising and recruiting materials, applications for protocol revisions, reports of any adverse consequences connected with the research project, and all IRB correspondence related to the research project.<\/p><p>i)&nbsp;Database including the following: &nbsp;title of research project, name(s) of principal investigator(s), funding source (if any), date of approval and any continuing approvals, date marking the end of approval period, type of review (Expedited or Full Board), and current status.<\/p><h5 class=\"wp-block-heading\" id=\"h-2-retention-of-records\"><a id=\"g2\" name=\"g2\"><\/a>2.&nbsp;Retention of Records<\/h5><p>IRB records are kept&nbsp; What kinds of studies need IRB approval?<\/h4><p><u>Any<\/u> research conducted using human participants must be submitted for review (but, see question #3 below).&nbsp;The submission entails filling out either a review form or a request for exemption.&nbsp; Please keep in mind that research includes <u>surveys and interviews<\/u> as well as other types of data collection.&nbsp;All Master&rsquo;s theses must be reviewed.<\/p><p>The Office of Human Research Protections provides a<a href=\"http:\/\/www.hhs.gov\/ohrp\/policy\/checklists\/decisioncharts.html\"> useful chart<\/a> to assist researchers with IRB questions concerning their research.<\/p><h4 class=\"wp-block-heading\" id=\"h-2-what-about-journalism-oral-histories-or-biographies\">2. What about journalism, oral histories, or biographies?<\/h4><p>Because biographers, journalists and historians are often working on research that is particular to the individual or individuals they are studying, their activities are outside of the purview of IRB and do not require review.&nbsp;In other words, because the research is not meant to be generalized outside of a specific person or situation, it does not meet the definition of research as set forth in the federal guidelines.&nbsp;Oral histories have recently been excluded from oversight by the IRB and no longer require review as they have in the past. If you are uncertain about your project, please contact the IRB.<\/p><h4 class=\"wp-block-heading\" id=\"h-3-i-am-collecting-data-from-my-class-to-determine-if-students-like-a-new-teaching-method-do-i-need-to-go-through-the-irb-to-collect-these-data\">3. I am collecting data from my class to determine if students like a new teaching method.&nbsp;Do I need to go through the IRB to collect these data?<\/h4><p>The federal guidelines define research as &ldquo;a systematic investigation&hellip;designed to develop or contribute to generalizable knowledge&rdquo;.&nbsp;The Department of Health and Human Services does not consider a study research unless you plan on disseminating the information you collect to a wider audience (e.g. through presentation or publication).&nbsp;Therefore, if you are collecting the data only to improve your course and will not be sharing the results, you do not need to worry about IRB approval.&nbsp;However, if you believe that you may present or publish your results, you need to submit an IRB form.&nbsp;(Most classroom research on educational strategies would require only a request for exemption).&nbsp;Make sure students know that refusing to participate will not affect their grades.&nbsp;The collection of data from your class can be seen as coercive, so it is important to be extremely careful in this area.&nbsp;It is always a good idea to keep anonymous any and all students who participated in your study.<\/p><h4 class=\"wp-block-heading\" id=\"h-4-what-about-student-research\">4. What about student research?<\/h4><p>This is a tricky one since federal and state policies do not specifically cover student projects. Because the results of projects are often not disseminated outside of the educational classroom setting, they technically do not fall under the purview of the IRB.&nbsp;Therefore, the University of Mary Washington&rsquo;s IRB does not require student projects conducted in courses to be reviewed if the purpose of these projects is only pedagogical in nature.&nbsp;If you have students who might present their research outside of the classroom or submit a paper for publication, they must go through the IRB. Although the IRB does not require review of classroom projects, it assumes that instructors in these courses provide close supervision of students, teach students the ethics of human research, and ensure that students do not perform research that is at all questionable in terms of ethics. If a student is dedicated to a class project that you believe should be reviewed, please feel free to submit it for review.<\/p><h4 class=\"wp-block-heading\" id=\"h-5-how-do-i-apply-for-approval\">5. How do I apply for approval?<\/h4><p>Your proposal will fit into one of three categories. It will be considered exempt, expedited, or in need of a full board review.&nbsp;After familiarizing yourself with the policies and procedures manual, fill out either a request for exemption or an expedited\/full board review form.&nbsp;The best thing to do is to look at the exempt form first.&nbsp;Listed on the form are categories of exempt studies.&nbsp;If your project fits into one of the exempt categories, fill out that form.&nbsp; If your study does not fit into one of the categories, you will need to fill out the more detailed expedited\/full board form. Remember to fill out the forms completely or they cannot be reviewed.<\/p><h4 class=\"wp-block-heading\" id=\"h-6-where-do-i-submit-the-form\">6. Where do I submit the form?<\/h4><p>Completed forms should be taken to the <a href=\"https:\/\/provost.umw.edu\/irb\/board-members\/\">appropriate IRB Board member<\/a> for review. The requestor will be notified via email if changes need to be made or if the project has been approved as is. All IRB requests will be forwarded to the Dean&rsquo;s office of the College of Arts and Sciences to be archived.<\/p><h4 class=\"wp-block-heading\" id=\"h-7-how-long-with-the-review-take\">7. How long with the review take?<\/h4><p>If your study falls into the exempt or expedited categories, the process will usually be completed in less than two weeks. If your study requires review by the full board (i.e. your study does not meet the expedited criteria), the review could take a month to 6 weeks.<\/p><h4 class=\"wp-block-heading\" id=\"h-8-can-i-start-collecting-data-before-i-receive-approval\">8. Can I start collecting data before I receive approval?<\/h4><p>Absolutely not. You must have approval before any data are collected.<\/p><h4 class=\"wp-block-heading\" id=\"h-9-how-will-i-know-that-my-research-has-been-approved\">9. How will I know that my research has been approved?<\/h4><p>You will receive an email from the chair of the IRB or designee indicating that you have been approved.&nbsp; IRB approvals will be good for one year from the date of approval unless you state on your application that the data collection will last longer than one year.<\/p><h4 class=\"wp-block-heading\" id=\"h-10-federal-regulations-require-that-all-researchers-receive-training-in-ethics-how-do-i-meet-this-requirement\">10. Federal regulations require that all researchers receive training in ethics. How do I meet this requirement?<\/h4><p>Faculty, staff, and students who wish to submit research for review by the UMW IRB must complete the online training program available at <a href=\"http:\/\/www.citiprogram.org\/\">www.citiprogram.org<\/a>.&nbsp;You will need to register for the course and select University of Mary Washington as the institution.&nbsp;Once you have completed the training, please retain a copy of the training completion certificate for your records.&nbsp;You will need to submit this certificate as proof of training with any IRB proposals.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>FAQs1.
